ROOTED Survey

ROOTED is asking parents and youth to take a few minutes to fill out a survey!  It is our desire to meet the needs of our youth and their families as best we can. But to do that, we would like to hear from you!  Even if you do not attend ROOTED but are youth age or have youth aged kids, please take a few minutes to let us know your thoughts.
